Not only was an overdose of propofol likely to blame for Joan Rivers's death, her doctors didn't have the proper equipment to deal with just that type of emergency.

As we reported yesterday, investigators in New York say Rivers stopped breathing during an endoscopy because doctors had administered too much propofol to put her under sedation. But now TMZ reports that the Yorkville Endoscopy Center wasn't prepared for that, either. There was no crash cart in the room, and had doctors had a drug called Succinylcholine handy, they could have relaxed her trachea -- which would have allowed her to breathe again.
